Cross Chat - for those who have multiple servers

Small changes to the CrossChat addon:

• formatting update
• the config file has now defaults
• for nerds: there are now hooks called crosschat.svreceive and crosschat.clreceive.

the only argument is a table. if you return true on the hook's function, the default code will not be executed.
